To receive a more accurate and final quote, please have your documents ready when filling out this form. All signers and witnesses must be present at the scheduled appointment.
⚠️ Last-minute reschedules or no-shows will void any refunds. You may re-book the same service within 30 days for 50% off.
Bring these every time so your appointment runs smoothly:
- A valid, unexpired government-issued photo ID for every signer (driver’s license, passport, etc.)
- The unsigned document(s) to be notarized (no blanks or missing pages)
- Any witnesses required (they must bring valid ID as well)
- Any additional instructions from the receiving institution (ex. number of copies, seals, or witnesses required)
